Only fully completed and signed registration forms will be accepted. The registration will only be confirmed upon receipt of the payment and the proof of status form. If the registration form and/or the full payment have not been received before the deadline indicated, the registration will be cancelled OR the next registration fee will automatically be charged.

All registration and course fees are exempt of German VAT. Payment by bank transfer is only possible until 31st August 2017 24:00 CET.

The official congress language is English and no translation will be offered. There will be sessions in German language.

1. Contract conclusion
Upon registration a contract is validly concluded with the Organiser regarding your participation in the EAOF Conference including all the additional services you have booked.
The additional hotel booking service is a service provided by UHHMG.
2. Remuneration and due date
2.1. Participation fee
Payment for all the services booked is due after receipt of the payment request/invoice. Please transfer the entire amount, consisting of congress fees, fees for the accompanying programme if booked, to the specified account within the specified period with all charges paid.
The amount of the participation fee will be debited from your credit card or EC card at the next possible date.
Pursuant to Section 4 No. 21 and No. 22 German Turnover Tax Act (UStG), the participation fee and the fees for the accompanying programme are exempt from VAT.
Please inform us upon registration if you have any particular requests concerning invoicing. Your registration will be confirmed via email. This email only has declaratory effect. The only decisive factor for concluding the contract and the due date of your payment is your registration to attend the congress.
2.1.1. Reduced participation fee
By registering at a reduced participation fee, the Participant assures that he/she meets the relevant registration criteria (student/representatives of patients or relatives associations, pensioners) at the time of participating in the congress. Upon UHHMG’s request, evidence thereof shall be presented. Otherwise, the full participation fee for the congress shall be paid. Such cost may also be invoiced additionally in retrospect.
To register as a student, representative or pensioner the required proof of status letter must be forwarded by the published deadline (together with the registration fee). A Proof of Status is an official letter written by the head of department – hospital, company or academic institution, which confirms the status of the applicant. The document must be issued in English on the organization’s official letterhead. When letters, certifying status, are sent after the early bird/standard registration deadline, the next registration fee will be applied even if the online registration was completed within the deadline.

2. 2. Hotel
Please submit your credit card details to UHHMG if your booking includes a hotel reservation; such data will only be used as booking security. The card will not be debited in advance by UHHMG and will only be used in case of no-show. The hotel and any additional services booked in connection therewith shall be paid upon arrival at the hotel.
Guests, who guarantee their hotel reservation by means of a credit card, agree that UHHMG is entitled to pass on the credit card details as a booking guarantee to the hotel upon the latter’s request. UHHMG does not store the credit card details electronically.
UHHMG will negotiate allotments with hotels and other providers of accommodation at special conditions for the Participants of the congress. The Participants have no claim to being provided with a hotel room during the desired period. The general terms and conditions and cancellation periods and conditions of the selected hotels or providers apply. These are available on the websites of the hotel that has been booked. The accommodation agreement is concluded directly between the Participant and the hotel or provider selected by such Participant. UHHMG only acts as an agent in this respect. All the claims and obligations resulting under the accommodation or travel service agreement exist directly and exclusively between the Participant and the selected hotel or provider.
The hotel room reservation can only be guaranteed and made after receipt of the requested credit card details. All requests regarding hotel reservations must be available to UHHMG. Special requests (such as rooms on the ground floor, smoking rooms, rooms equipped for disabled persons, etc.) must be expressed in writing and we will be pleased to pass them on to the relevant hotel. The hotel will consider such requests as far as this is possible. Please note that hotel reservations will be automatically cancelled if they were not confirmed by submitting the credit card details within the period specified in the booking confirmation.

3. Cancellation
Changes and cancellations must be notified to the Organiser in written or electronic form, thereby stating the name, address and registration number. The notification shall be addressed as follows:

The following cancellation terms apply:
3.1. Participation fee
The registration for the congress is binding. Following registration, 100% of the fee is payable, even if the registration is cancelled or the Participant does not attend. Exceptions to this rule must be agreed with UHHMG in writing and the decision will be taken by the Organiser on a case-by-case basis.
The Organiser will accept a substitute participant, provided such substitute agrees to take over the arrangements made by the Participant in full.
UHHMG will charge a handling fee in the amount of EUR 25.00 incl. 19% VAT for changing/rebooking the relevant documents.
Rebookings only become effective after this amount has been paid and a confirmation has been received.
3.2. Accompanying programme
Cancellations regarding the accompanying programme may be made without charge until 01 June 2017.
After 01 June 2017 fallen 100% of the fee must be paid. Participation is at your own risk and subject to the insurance cover of the provider.
3.3. Hotel reservations
Changes and cancellations must be notified to UHHMG in written or electronic form, thereby stating the name, address and registration number. If a complete booking or individual nights are cancelled, the hotel may charge up to 100% of the rate for each cancelled night. In this case, the credit card details that were submitted as a guarantee will be used to debit the credit card or any advance payments made will be kept. Later arrivals or earlier departures are also deemed cancellations and may be charged by the hotel. The general terms and conditions of the hotel booked shall apply; see also item 2.2.
Bank charges incurred by changes to or cancellations of hotel bookings shall be borne by the guest.
3.4. Cancellation of the event
The Organiser reserves the right to cancel the congress if there are not enough participants or as a result of force majeure. In any case, we will seek to inform you as early as possible. In the event of such a cancellation any fees already paid, will naturally be reimbursed. Any claims beyond this scope are excluded.
4. Liability
The liability of the Organiser and its vicarious agents for any damage that is not bodily harm is limited to three times the price of participating/the invoice total, provided such damage was caused by neither intent nor gross negligence and to the extent that the Organiser is not only responsible for any damage incurred by a Participant due to the fault of a sub-contracted service provider. The Participant is responsible for organising transportation to the event venues and the hotels at his/her own expense. The liability of the Organiser and its vicarious agents is limited to cases of grossly negligent and intentional breach, unless damages to life, limb or health are concerned or material contractual obligations are breached. The Organiser assumes no liability for damages that the Organiser suffers due to third parties and for which UHHMG is not responsible. Any claims that the Organiser may have vis-à-vis third parties shall be assigned by the Organiser to the Participant upon request. Liability of any kind whatsoever for the validity and enforceability of the assigned claims is excluded.
The Organiser is responsible for the scientific programme, the overall event and the accompanying programme.
The participation fee is collected in trust by UHHMG on behalf of the Organiser. The bookings are subject to the above General Terms and Conditions for registering for the congress. Certificates of attendence/congress tickets are not issued before payment has been made in full. The Organiser assumes no responsibility for lost or unused documents/certificates of attendance/badges (tickets). Reissue of such documents is subject to a charge of EUR 10.00 incl. 19% VAT.
5. Privacy policy
The German Federal Data Protection Act (BDSG) is strictly observed when storing and processing personal customer or Participant data. Personal customer/Participant data is stored and processed only to the extent that this is required for implementing and handling the contractual relationship with the customer and as long as the Organiser is obliged by law to store such data. In order to register for the congress stated above, it is unavoidable that your personal data is collected, stored and processed. This is done solely for the purpose of organising and implementing the event. Your data is only passed on to third parties that are directly involved in the running of the congress and if the organisation thus requires (parties in charge of implementing the congress, hotel).
6. Footage/photographs
Please note that film, photo and audio recordings may be made of all items on the programme agenda associated with this booking within the scope provided by law (see Section 23 German Act concerning Copyrights to Works of Art and Photography (KunstUrhG)). These may be processed, used, exploited and published without limitations regarding time and region by the Organiser and by UHHMG free of charge in the scope of publicity in for example picture galleries, reviews and illustrations. Upon registering, the Participant agrees to such use.

9.1. If the written form is prescribed in these Terms and Conditions, notifications sent by email or fax are deemed to meet such requirement.
9.2. German law, excluding international private law and the United Nations Sale of Goods Convention (CISG) that was adopted into German law, shall apply to all legal relationships resulting from the registration to participate in events offered by the Organiser and/or UHHMG.
9.3. The official language of the contract is German.
